FALL 2022 Application for On-Campus Program

DIRECTIONS FOR NEW FRESHMEN OR TRANSFER STUDENTS

Steps to begin the SALT Center Application for students new to the University of Arizona:

1. Navigate to the New Freshman or Transfer Student application on the SALT Center website.

2. Sign in with the Future Wildcat credentials that were created to apply to the University of Arizona.

4. Select ‘SALT Center’ for Application Type.

5. Select ‘Fall 2022’ for the Term.

6. Select the ‘Start New Application’ link.



  • Apply to the SALT Center immediately to avoid a waiting list
  • Set aside at least 30 minutes to complete the application
  • The application may be saved at any step to come back to later
  • The most important step is to fill out the application completely and submit it
  • Once your application is complete, the SALT Center Admissions Team will review it to determine your eligibility to enroll

After you are notified of being eligible to enroll in the SALT Center, two mandatory steps are required to finalize enrollment:

  1. Complete a SALT Center New Student Participation Agreement
  2. Attend a SALT Center Enrollment Event

Historically, April 1st has been the date that the SALT Center has reached capacity and initiated a waiting list.  SALT Center candidates are encouraged to submit a complete application and complete our enrollment steps by this date in order to have the highest chance of avoiding the waiting list.

Commit now, and then notify the SALT Center at dhaymore@email.arizona.edu if your plans change.


Please direct any questions about accessing the SALT Center application to Dennis Haymore at dhaymore@arizona.edu.

Please include the candidate's first and last name in the email subject line. Please allow up to 48 hours for a response. If the email comes from someone other than the candidate, they will always be included when necessary and warranted.

1. Navigate to the Continuing UA Student application on the SALT Center website.

2. Log in with your Net ID

3. Select ‘SALT Center Current Student’ for Application Type

4. Select ‘Fall 2022’ for the Term

5. Select the ‘Start New Application’ link

HOW TO UPLOAD YOUR DOCUMENTATION

We hope that you find these step by step instructions helpful in uploading your documentation to your application.


1. Please do not submit documentation from prior to high school.

Documentation from prior to high school is not useful for our review process. Students must qualify for our services and we need to ensure that our services match the student’s academic support needs; current/comprehensive documentation best allows us to make this determination.

2. Please do not password protect your documents.

This will cause a significant delay during the application review process.

3. Why am I being asked for documentation?

Documentation is the comprehensive testing that led to a diagnosis of a learning or attention challenge. Providing documentation of your learning or attention challenge is a key step to submitting your application in order to be considered eligible to enroll. 

 

Do you have a preferred format for my documentation?

Documentation must be uploaded to the application in electronic format.  The file must be a Word or PDF document. 

Is there a file size limit?

Yes. The maximum file size you can upload is approximately 20 MB (megabytes).

I do not have documentation of a learning or attention difference, what must I do to satisfy this requirement?

If you do not have documentation, or have never been formally diagnosed with a learning or attention challenge, you will be prompted to answer a simple essay question:

"Describe your history of academic challenges and the support services you have used to manage these challenges."

How current must the documentation be?

The SALT Center would like to see what you consider the most current and the most comprehensive.  The SALT Center requests that you submit paperwork from your high school career.

Some examples of documentation include:


Psychoeducational Evaluation

This is the comprehensive battery of testing that led to a diagnosis of a learning or attention challenge. 

Psychoeducational testing refers to the psychological tests used to analyze the mental processes underlying your child's educational performance. As part of the assessment, your child will undergo psychoeducational testing.

Preferred
IEP An Individualized Education Program (IEP) is a written statement of the educational program designed to meet a child’s individual needs.

The IEP has two general purposes:

  • to set reasonable learning goals for a child
  • to state the services that the school district will provide for the child

504 Plan The 504 Plan is a plan developed to ensure that a child who has a disability identified under the law and is attending an elementary or secondary educational institution receives accommodations that will ensure their academic success and access to the learning environment. This is okay, however, we may need to contact you for additional information
Disability Statement This is a general statement from a physician, licensed clinical psychologist, or psychiatrist that a diagnosis of a learning or attention challenge is present. This is okay, however, we may need to contact you for additional information

The following three examples will guide you through uploading your documentation

Scenario 1 - You have a documented learning or attention difference, and you will be submitting documentation to us right now.

  • Answer 'YES' to both questions
  • Click the link beneath the second question
  • Upload documentation in Word or PDF format
 

Scenario 2 - You have a documented learning or attention difference, and you will not be submitting documentation at this time.

  • Answer 'YES' to the first question
  • Answer 'NO' to the second question
  • Review the message that will pop up

Scenario 3 - You do not have a documented learning or attention difference, and therefore have no documentation to upload.

  • Answer 'NO' to the first question
  • You will be asked to submit a response to the following question:

"Describe your history of academic challenges and the support measures you have used to manage these challenges."

 

FINANCIAL AID INFORMATION

  • Include the SALT Center program fee as part of the Cost of Attending (COA) the University of Arizona
  • Complete and submit the SALT Center Scholarship Award application by March 1 to be considered for a need-based award 
  • File a FAFSA (Free Application for Federal Student Aid) by March 1 with the UA Office of Scholarships and Financial Aid
